Box 413—00200, Nairobi in the Republic of Kenya, on behalf of Michelle Wangechi Murimi (minor), formerly known as Michelle Wakini Murimi, formally and absolutely renounced and abandoned the use of her former name Michelle Wakini Murimi and in lieu thereof assumed and adopted the name Michelle Wangechi Murimi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Michelle Wangechi Murimi only.
