Box 206, Shinyalu in Kenya, formerly known as Bramilla Musasia Fashek, formally and absolutely renounced and abandoned the use of his former name Bramilla Musasia Fashek born in Kilifi South District, and in lieu thereof assumed and adopted the name Bramilla Musasia born in Kakamega District, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Bramilla Musasia only.
