Box 7072– 01000, Thika in the Republic of Kenya, on behalf of Alisha Delanie Imani (minor), formerly known as Allisha Indakuli Mukabale, formally and absolutely renounced and abandoned the use of her former name Allisha Indakuli Mukabale and in lieu thereof assumed and adopted the name Alisha Delanie Imani, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Alisha Delanie Imani only.
