Box 5431-80200, Malindi in the Republic of Kenya, formerly known as Alice Kinyori Muthoni, formally and absolutely renounced and abandoned the use of my former name Alice Kinyori Muthoni and in lieu thereof assumed and adopted the name Alice Muthoni Ann, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Alice Muthoni Ann only.
