Box 381-80108, Kilifi in the Republic of Kenya, on behalf of Laura Mkanjala Mwangi (minor), formerly known as Laura Mkanjala Mwasingo, formally and absolutely renounced and abandOned the use of her former name Laura Mkanjala Mwasingo and in lieu thereof assumed and adopted the name Laura Mkanjala Mwangi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Laura Mkanjala Mwangi only.
