Box 1270, Garissa in the Republic of Kenya, formerly known as Sahara Mohamed Shide, formally and absolutely renounced and abandoned the use of my former name Sahara Mohamed Shide and in lieu thereof assumed and adopted the name Sahara Ahmed Seylic for all purposes and authorizes and requests all persons at all times to designate describe and address me by my assumed name Sahara Ahmed Seylic only.
