Box 3295–00200, Nairobi in the Republic of Kenya, formerly known as Johnson Mutua, formally and absolutely renounced and abandoned the use of his former name Johnson Mutua and in lieu thereof assumed and adopted the name Jackson Mutua, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Jackson Mutua only. That I absolutely renounced and abandoned the use of my date of birth as 5th May, 1963 appearing in my identity card and assumed and determined and taken my date of birth as 25th May 1964 as appearing in my birth certificate.
